Your family’s information.

The tryout form is for parents and guardians. ASCEND collects the adult’s name, email and phone number, plus the player’s first name, birth year, age group and experience, to organize tryouts and respond to your interest.

Coach-invited families can create a parent account. The team portal stores account details, selected players and tournament registrations so coaches can manage the season. Parent-completed player registration includes full name, date of birth, uniform sizes, parent and emergency contact information, allergy details, and optional dietary or care notes. These details help coaches prepare for team activities. Your player’s information is not shown on the public homepage.

Each adult uses a separate account. A coach can connect multiple children to one parent or give another parent or guardian access to selected children. Approved adults linked to a player can view and update that player’s registration, emergency contacts, care details, and tournament registrations. Linked adults can see which parent accounts have access to the children they share. Contact your coach to review or remove additional guardian access.

Sponsorship inquiries collect an adult’s name, email, optional organization and phone number, and details about the support they would like to offer. ASCEND coaches use this information to follow up and organize sponsorship conversations. The inquiry form does not collect payments or card details.

Information is available to authorized ASCEND coaches, approved adults linked to the player, and the service providers that host the website and its data. We do not sell your family’s information or use the tryout form to enroll you in unrelated marketing.

When a parent marks a team update as read or acknowledges it, ASCEND stores that response and the message version for that adult’s account. Authorized coaches can see which parents and guardians have reviewed the current information. When an adult signs a team document, ASCEND retains the exact form version, the player linked to that form, the typed signature, account email, relationship attestation, photography choice when applicable, consent statements, and review and signing times. Signing requires the adult’s current password; the signing record does not store that password. Original forms and signed PDFs are private and available to the signer and authorized coaches. Another guardian does not automatically receive that adult’s signed copy. A signer can retain access to their own completed records after player access is paused or guardian access is removed. Contact support@ascend-oc.com for paper options, records, corrections, or a change to future photography permission. Existing signed records are retained as records of the version and choices made; a new version requires a new signature. New assignments appear inside the portal; they do not automatically send email or text messages. The portal uses an essential sign-in cookie. There are no advertising trackers on this website. Calendar downloads include tournament details, not player or family details.

Ask your ASCEND coach to update or delete your details, withdraw tryout interest, or help with account access. Keep care notes limited to information your coach needs for team activities. Do not upload medical records, insurance numbers, school details, or identity documents. Your player registration can be updated in the parent portal; speak directly with your coach about an urgent change or a detailed care plan.
